A Communication Tool

Communication between providers is especially important in prenatal care. Prenatal care is often delivered by multiple providers including physicians, midwives, nurse practitioners, nurses, and residents. Since each encounter then literally becomes a “new” encounter, it is crucial that the communication tool impart precisely what has been done before and what needs to be done next for any one patient.

As one moves from a solo practice to a larger community of providers, communication often becomes more muddled. It is not uncommon that even within single-specialty groups, several different paper prenatal forms are utilized.
